Central London Community Healthcare NHS Trust (CLCH) is looking for qualified Community Nurses. There are opportunities to work in the London Boroughs of Merton, Barnet, Harrow, Hammersmith & Fulham, Kensington & Chelsea and Westminster.
We provide community health services to around two million people across London and Hertfordshire. Every day, our staff provide high quality healthcare in people’s homes or at local clinics, helping them to stay well, manage their own health and avoid unnecessary trips or long stays in hospital.
The Community Nurse will be required to provide nursing care, working under the supervision of the District Nurse Team Lead to patients in a variety of community settings. They will undertake community nursing duties involving delivery of nursing services and holistic assessments of patients health needs using a person centred approach. This includes providing quality nursing care to housebound patients and being proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long term care institutions. The post holder will also be involved in facilitating self-care for patients with long term conditions, health promotion and innovative activities and projects that enhance patient service delivery.
The ideal candidate for this role will be a Registered Nurse with current NMC registration. They will have gained recent clinical nursing experience and have had exposure to working with people with complex health needs. The ideal candidate will also have previous experience in the Nursing process, care planning and be fluent in record keeping.
